Telehealth Informed Consent
This document explains what it means to receive healthcare services through the Pepteeva Health telehealth platform. By creating an account, completing an intake assessment, or participating in a telehealth consultation, you acknowledge and consent to the terms below.
Please read this carefully. Telehealth is a convenient way to receive provider-led care, but it is not identical to an in-person visit and is not appropriate for every condition. All clinical decisions on the Pepteeva Health platform are made by independent licensed healthcare providers.
1. What Is Telehealth?
Telehealth is the delivery of healthcare services using electronic communications and technology — including secure video, audio, messaging, and the exchange of medical records and health data — when you and your provider are not in the same physical location.
Telehealth services on our platform may include health assessments, provider consultations, diagnosis, treatment recommendations, prescription of medications where clinically appropriate, lab test ordering and review, and ongoing care coordination.
2. Scope of Our Telehealth Services
- Care is provided by independent licensed healthcare providers who are credentialed and licensed in the state where you are located at the time of your visit.
- Our services focus on wellness optimization — including peptide therapy, hormone optimization, weight management, and related programs. We do not provide primary care, emergency care, or treatment for severe or unstable conditions.
- Your provider determines, in their sole clinical judgment, whether telehealth is appropriate for you and whether any treatment or prescription is indicated.
- You may be required to complete lab work before starting certain programs.
3. Expected Benefits
- Access to licensed providers without travel or in-person visits.
- Convenient scheduling and asynchronous messaging with your care team.
- Continuity of care through ongoing monitoring, check-ins, and protocol adjustments.
- Efficient coordination of lab testing and prescription fulfillment.
4. Potential Risks & Limitations
As with any healthcare service, telehealth carries potential risks, including:
- Absence of physical examination: Your provider cannot perform a hands-on exam, which may limit their ability to diagnose certain conditions. Information you provide may be a substitute for direct examination.
- Technology failures: Interruptions, delays, or failures of electronic equipment or connectivity may delay care.
- Information gaps: If you provide incomplete or inaccurate information, clinical judgment may be affected and outcomes may suffer.
- Security risks: Despite security safeguards, in rare cases electronic transmission of information could be intercepted or fail.
- Results are not guaranteed: Individual results vary. No specific outcome of any protocol or therapy is promised or guaranteed.
5. Emergencies
Our platform is not for medical emergencies.
If you are experiencing a medical emergency — including chest pain, difficulty breathing, severe allergic reaction, or thoughts of harming yourself or others — call 911 or go to the nearest emergency room immediately. Do not wait for a response through the platform.
6. Your Responsibilities
To receive safe and effective care through telehealth, you agree to:
- Provide complete, accurate, and current information about your identity, location, medical history, medications, allergies, and symptoms.
- Be physically located in the state you report at the time of each telehealth encounter.
- Follow your provider's instructions, including required lab work and follow-up visits.
- Report side effects, adverse reactions, or changes in your condition promptly.
- Maintain the confidentiality of your account credentials and use the platform only for yourself.
- Seek in-person or emergency care when your condition requires it.
7. Privacy & Security
Your health information is protected under federal and state law. We use secure, encrypted systems designed to protect your Protected Health Information (PHI) during telehealth encounters and while stored.

8. Prescriptions & Clinical Judgment
- Completing an assessment or purchasing a membership does not guarantee that a provider will issue a prescription or recommend any specific treatment.
- All prescriptions are issued solely at the discretion of an independent licensed provider, based on their clinical evaluation of your individual case.
- Certain therapies may involve compounded medications prepared by state-licensed 503A and 503B compounding pharmacies. Compounded medications are not FDA-approved. Your provider will discuss whether a compounded therapy is appropriate for you.
- You are free to decline any recommended treatment and to seek care elsewhere at any time.
9. Your Rights
- You may withhold or withdraw this consent at any time by notifying us in writing, without affecting your right to future care, though withdrawal may limit our ability to continue providing telehealth services.
- You may request an in-person referral or a copy of your medical records at any time.
- You have the right to ask your provider questions about your care, treatment options, and alternatives before consenting to any protocol.
- No provider-patient relationship is created until a licensed provider reviews your information and accepts you as a patient.
10. Acknowledgment & Consent
By using the Pepteeva Health platform, you acknowledge that:
- You have read and understand this Telehealth Informed Consent.
- You understand the benefits, risks, and limitations of telehealth described above.
- You have had the opportunity to ask questions, and your questions have been answered to your satisfaction.
- You voluntarily consent to receive healthcare services through telehealth.
- This consent will remain in effect for all telehealth services you receive through the platform unless you withdraw it.
